Social Worker — Subiaco Area, Perth Region

Your role at St John of God Subiaco Hospital St John of God Subiaco Hospital is seeking a skilled and compassionate Social Worker to join the hospital’s multidisciplinary Allied Health Team. In this rewarding role, you will provide high-quality clinical social work services to patients with complex health, psychosocial and discharge planning needs. Working collaboratively with patients, families, carers, medical specialists and allied health professionals, you will play a vital role in supporting recovery, independence and wellbeing. The role is offered on a Permanent Part Time basis, working 30.4 hours per week The Position: Deliver comprehensive social work assessment, intervention, counselling, advocacy and discharge planning to support patient-centred care and positive health outcomes. Collaborate effectively with multidisciplinary teams, patients, carers, community agencies and compensable bodies to coordinate services and facilitate safe transitions of care. Contribute to service excellence through quality improvement, professional development, student supervision and maintaining compliance with relevant legislation, policies and funding requirements. Maintains professional competence through ongoing training, completion of mandatory competencies, and active participation in continuing professional development. The successful candidate will hold a recognised tertiary qualification in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW). They will bring experience in social work assessment, case management and discharge planning, along with strong interpersonal, written and verbal communication skills and the ability to work effectively within multidisciplinary teams. A sound understanding of the social determinants of health, particularly in relation to Aboriginal health, together with knowledge of public and private funding arrangements, will be highly regarded. The successful applicant will also demonstrate a commitment to continuous quality improvement and be willing to work across multiple locations within Community Services as operationally required.
