Your new role: As a Social Worker within Metro South Palliative Care Service (MSPCS), you will deliver high-quality palliative care social work services to adults receiving end-of-life care and their families across community settings and inpatient units including Canossa Palliative Care Unit and Wynnum Manly Palliative Care Unit. Guided by the National Palliative Care Standards, the Australian Association of Social Workers Code of Conduct, and Palliative Care Social Work Standards, you will provide comprehensive psychosocial assessment, therapeutic intervention, care planning and bereavement support within a collaborative interprofessional environment. This role reports operationally through the MSPCS leadership structure and professionally through Social Work services at Princess Alexandra Hospital. Key Responsibilities * Deliver comprehensive psychosocial assessments and therapeutic interventions for palliative patients and families * Manage a clinical caseload across community and inpatient settings * Support discharge planning and care coordination within an interprofessional team * Address emotional, social, spiritual and bereavement needs * Maintain accurate documentation, reporting and service statistics About you: You are a compassionate and clinically capable Social Worker who thrives in complex, emotionally sensitive environments. About us: Metro South Palliative Care Service (MSPCS) provides specialist palliative care utilising the model for population-based care developed by Palliative Care Australia. MSPCS provides direct care in the Canossa Palliative Care Unit and Wynnum Manly Palliative Care Unit, consultative and/or direct care for patients in all hospitals in the Metro South Hospital and Health Service and provides community-based specialist palliative care across the district. Community services, including in-reach consultatory service to Residential Aged Care Facilities, are based at Brisbane South, Redland and Logan & Beaudesert.
