About The Role We are looking for a part-time Registered Nurse to join our welcoming and supportive Juniper Home and Community Care team in the Perth metropolitan area. In this rewarding role, you will provide care within the community, visiting clients in their own homes across the Metro region to deliver compassionate, high-quality support. Due to the expansion of the Juniper Community Home Team, we are seeking a Registered Nurse to join our South team. Part-time role, 32 hours per week (Monday, Tuesday, Wednesday & Thursday 0800-1600) supporting clients in the Perth metropolitan region. Candidates with previous experience working in Home and Community Care will be highly regarded. Key Responsibilities Assess and review customer needs in consultation with case managers Clinical Nursing assessments (PAS & KICKA) Develop, implement, and evaluate plans of care in partnership with customers and their significant others for both chronic and complex needs Simple & complex wound care Liaise with appropriate health care professionals to deliver quality care to customers Participate in multidisciplinary team meetings Ensure full compliance with organization, professional and aged care standards Key Requirements Current registration with AHPRA as a Registered Nurse Community experience highly regarded AlayaCare experience highly desirable Ability to relate to elderly customers with empathy whilst working effectively with other health professionals, service providers and case managers Strong clinical assessment skills Work autonomously with strong communication, organizational and time management skills Enthusiasm for professional growth and development Competency with IDCs, wound care and strategies for living with dementia – desirable Cultural awareness and sensitivity Current Australian driver’s license and reliable vehicle - both essential What’s On Offer Attractive hourly rate between $44.76 - $56.45 per hour* dependent on experience.
