About the role Temporary full time opportunity to deliver evidence based routine social work clinical services as a member of a multidisciplinary team to residents with complex mental illnesses in a specialised residential facility /rehabilitation unit, to facilitate rehabilitation, recovery, and transition to independent living. About you Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values: * As a member of the multidisciplinary team, deliver contemporary routine clinical Social Work services to individuals and groups of consumers with complex mental health and alcohol and other drug treatment needs based on valid and reliable evidence and commensurate with level of experience, in accordance with professional practice, ethical and mental health standards to ensure culturally appropriate consumer focused outcomes. * Actively participate in the multidisciplinary team by providing routine Social Work advice (including advice relating to child protection and family violence concerns/family sensitive practice/NDIS) and contribute to the development of individualised treatment plans including discharge planning and service provision. * Participate in the identification, development, implementation and evaluation of quality, service improvement and/or research activities that enhance and improve consumer care, and contribute to the development of an innovative Mental Health Service.
