Your new role: As a Senior Social Worker, you will deliver advanced social work services to patients and families in GEMRHITH and Day Therapy services at QEII Jubilee Hospital. You will support complex care needs, contribute to evidence-based practice, and provide leadership in clinical education, supervision, and service development. Key responsibilities include: * Delivering evidence-based social work assessments, interventions, and discharge planning * Applying advanced clinical judgement to address complex psychosocial needs * Providing supervision, education, and support to HP3 staff, students, and AHAs * Contributing to quality improvement, service innovation, and research initiatives * Liaising with patients, families, carers, and community services to ensure safe transitions of care * Supporting interdisciplinary team processes and patient-centred care planning About you: We are seeking a compassionate and motivated senior clinician who thrives in rehabilitation and aged care environments.
