The Social Work Service includes approximately 52 EFT (operational reports) with an additional 70 EFT (professional reports) who provide interventions to inpatients and outpatients across all Alfred Health clinical programs. The Social Work Service is aligned to the Alfred Health strategic goals and operational performance targets. The Social Work Service is committed to the teaching and ongoing professional education of staff and students, research and quality improvement activities. All Social Work staff must meet the profession’s competency-based minimum standards of practice at all times. All Social Work staff must be eligible for AASW registration. The Alfred Health Social Work Service provides the workforce to each clinical program and therefore supports each clinical program meeting performance targets including patient experience, access, elective surgery and waitlist management measures. The Role This is an exciting new role for a Grade 2 Social worker to assist Alfred Health in supporting patients within the Trauma Stream and designated clinical units at The Alfred Hospital. The Social Worker is responsible for identifying and meeting the needs of the specialist patient group in collaboration with the clinical units, particularly the management of complex clinical cases. Excellent interpersonal skills are essential for this position, including negotiation and communication skills, both written and verbal. An ability to display flexibility within work practice and to work as part of a multi-disciplinary team are also essential. This role will require collaboration with a wide range of internal and external stakeholders is an important components in this position. The worker will be required to consult with staff, both Social Workers and those from other disciplines, on issues relevant to patient care and provide comprehensive case work expertise. Consultation with the NDIS, Workcover, ACAS, Transitional Care Program (TCP), Carer Services, Office of the Public Advocate (OPA), Victorian Civil Administrative Tribunal (VCAT) will also form a key component of this role and knowledge of these community services will be essential. Skills and Qualifications Required Eligibility for membership of the Australian Association of Social Workers. Bachelor of Social Work, or recognised equivalent. Experience in an acute and or sub acute hospital setting is preferable Ability to work effectively in a multi-disciplinary team and with families, exhibiting initiative and facilitative qualities Knowledge of NDIS, Aged Care services and TAC funding models will be advantageous Knowledge of working within an Electronic medical record will be required for this role Victorian Drivers Licence Key Responsibilities: Provide clinical supervision to Grade 1 Social Work staff as per discipline and AASW Guidelines. Provide a mentoring role for Grade 1 Social Work Staff. Participate in supervision from more senior staff member as per discipline and AASW Guidelines and participate in Professional Development, Work Planning & Annual Discussions. Actively engage in Professional Development Plans and model a commitment to learning. Actively participate in internal and external continuing education. Ability to take on a Social work portfolio within the department Access information as necessary to improve knowledge and skills. Supervise undergraduate Social Work Students through the Students and teaching program Participate in the development, implementation and review of social work student education in conjunction with senior staff. Demonstrate the ability to assume extra responsibilities as requested by the supervisor or senior clinicians. Assist in raising the profile of the Social Work role within Alfred Health Participate in continuous quality improvement activities and research activities both within the clinical team and department as required.
