See more at our website - www.atsichsbrisbane.org.au Job Description About The Role As a key member of the ATSICHS Brisbane team, The Social Health Care Coordinator effectively assesses, triages and refers clients who present, or are referred to the Social Health team. The role coordinates the intake system by screening and assessing the needs of families and individuals while operating within a strengths and priority-based framework. When assessed as being required, the care coordinator will deliver case management for clients, providing a range of assessed social and wellbeing supports to clients in meeting their holistic needs. Supports will be goal driven and focused on building the resiliency and capacity of individuals. Duties Provide effective first point of engagement with families and individuals in a welcoming manner, assess/screen their suitability for Social Health services, and, with the support of the team, allocate them for the appropriate service (case management, counselling or psychology) Facilitate case discussion within the Social Health team and with the Primary Health team particularly with respect to incoming referrals management to ensure clients are well supported and practitioners support one another. Assessments (screening, risk, intake, ATOMIC, and IUIH Social Health Care Pathway) are completed with client using the designated Social Health tools and practice approach. Provide brief mental health interventions and de-escalation of crisis episodes within medical clinics, working closely with internal Social Health and Primary health teams. Work to foster productive working relationships with relevant government departments, hospitals, key providers and other specialist providers including network with relevant services and practitioners within the region. Work as part of a multi-disciplinary team including Psychologists, Counsellors, GPs, Aboriginal Health Workers and Allied Health staff. Desired Skills and Experience Required Bachelor qualification in either Social Work/ Human Services/ Behavioral Science/ Social Science/ Psychology/ Criminology/ Allied Health/ AOD/ Occupational Therapy/ Sociology/ Public Health or relevant field. Demonstrated ability to apply a high level of practice judgement when working with clients in setting and achieving goals. Ability to work with a culturally and socially diverse group of clients, and to apply a culturally responsive framework to guide interventions, with the support of staff and management. Ability to work with diverse people and presentations in providing case managed SEWB supports. Demonstrated ability to record accurate and consistent case notes. Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services. National Police Certificate Working with Children's Blue Card or willing to apply. Working with Disability NDIS Yellow Card. Driving Licence. Evidence of Work Rights. Preferred Previous experience in a multi-disciplinary team environment. Previous experience working in an Aboriginal Medical Service.
