This role offers the flexibility of part-time or full-time hours and involves delivering psychosocial care and facilitating discharge planning for inpatients. The successful candidate will also contribute to professional and service development , including mentoring junior staff , supervising students, and supporting evidence-based practices across a multidisciplinary team. There is scope to work across palliative and rehabilitation services as needed. Key Responsibilities Deliver high-quality, patient-focused psychosocial care Build collaborative working relationships with patients, families, and hospital staff Use strong conflict resolution and negotiation skills to support patient outcomes Manage a complex caseload efficiently and coordinate timely hospital discharges Supervise and support Grade 1 Social Workers, students, and Allied Health Assistants Engage in professional development and lead innovative service improvement initiatives About You You are a motivated and compassionate social worker who thrives in a multidisciplinary healthcare environment.
