The District Nursing Service provides holistic care to people in their homes and clinic settings aimed at providing person centred care, ensuring patients receive treatment at home that keeps hospital admissions and readmissions to a minimum. The District Nurse empowers patients and their carers to develop goals and be actively involved in their own care. About You To be successful in this role you will need to be a registered nurse with a minimum of three years experience nursing in a clinical environment and/or a community setting, particularly relating to wound care, palliative care, gerontology, continence, community services and an understanding of aged care issues. You will need to be self-directed with good time management, communication and interpersonal skills. You will also need to demonstrate that you can provide high quality care in a complex and changing environment as well as experience and confidence using information technology, including mobile devices.
