About the Role Join our team as a Grade 1 Social Worker on a full time, fixed term basis until December 2025, to work at Monash Medical Centre based in Clayton. Travel to other Monash Health sites may be required. As a Grade 1 Social Worker, you will provide patient-centred psychosocial support through assessment, counselling, intervention, discharge planning, and advocacy. Working as part of a multidisciplinary team, you will manage your caseload using evidence-based practice and clinical reasoning while supporting equitable access to care. You will contribute to a safe and inclusive care environment by integrating ethical, cultural, and digital competencies into your practice. The role also includes supporting students and less experienced staff, participating in quality improvement initiatives, and embracing digital health solutions and professional development opportunities. About Us The Social Work Service at Monash Health assists patients and families with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning. Social Workers function as members of the multidisciplinary team and have a pivotal role where social complexity exists. The Social Work Service operating hours are standard business hours Monday to Friday. A weekend and on call service is provided by roster at all acute sites (Grade 1 staff are not required to participate in the on call program). We invest in working with undergraduate social work students under guidance from Senior Social Workers/designated clinical supervisor. About You You are a compassionate and committed Social Worker who brings a strong foundation in clinical care, advocacy, and psychosocial support. With the ability to semonstrate an understanding of key social work theories and confidently apply them in practice, your are able to work collaboratively with multidisciplinary teams to deliver safe, evidence-based care that supports patients, families, and carers across diverse and complex health settings. You are skilled in psychosocial assessment, counselling, discharge planning, and case coordination, and you navigate service systems such as Family Violence, Child Protection, Mental Health, NDIS, and My Aged Care with confidence. You value ethical practice, cultural sensitivity, and uphold the principles of recovery-oriented care. As an early-career clinician, you are eager to grow your capabilities in leadership, digital health, research, and quality improvement. You are reflective in your practice, embrace innovation, and actively contribute to service development. You are committed to continuous learning, uphold Monash Health’s values, and demonstrate professionalism, integrity, and accountability in all that you do.
