Apply person-centered approach and a solution focused and strength approach. Coordination and management of the day-to-day operations in our Supported Independent Living program. Coordination and development of clients' care plans in liaison with families and other stakeholders. Maintaining of participants records and providing updates on progress in weekly, monthly or quarterly reports. Coordinate and oversee rostering and timesheet requirements according to client support needs. Provide supervision and support to team members. Provide daily care and supervision to clients. Identifying and investigating discrepancies between bank statements and internal records. Skills and Experience Required: Previous Experience in Disability or Community Services Working with People with Diverse Disabilities Delivering care aligned with client-specific goals and NDIS requirements Demonstrated experience in delivering services that are tailored to individual goals, preferences, and abilities. Experience assisting with domestic tasks. Supporting individuals to access community services, recreational activities, and social outings.
