About the Role: Fix-Term, Part-Time role providing comprehensive social work services to patients, families and carers in our St Vincent’s NELUNE Men’s Health Centre. Conduct psychosocial assessments for patients and assess issues of risk As the sole social worker, you’ll work with other allied health clinicians and prostate cancer nurse to provide supportive outpatient services to prostate cancer patients who are at various stages of their cancer survivorship, attend weekly MDT meeting, involve in quality improvement initiates and participate CPD with Oncology social worker department at Kinghorn cancer centre. Provision of counselling, individual and family, to facilitate adaptation to medical conditions, lifestyle implications and issues of anticipatory grief and loss. Advocate for patients, families and carers as appropriate including where required, applications to NCAT: Guardianship Division and the National Disability Insurance Scheme (NDIS), Housing NSW, Department of Home Affairs, My Aged Care (MAC), LTCS, facilitate CTP claims and other Government and NGO entities as required. About You: Degree in Social Work and eligible for membership of the Australian Association of Social Workers (AASW) with wealth of clinical experience in a hospital or health related field as a practising Social Worker. Skilled in delivering comprehensive psychosocial care, including assessments, counselling, therapeutic interventions, case management, advocacy, and discharge planning — always with a person-centred and strengths-based approach. Specialised knowledge and hands-on experience in oncology and prostate cancer care, supporting patients in outpatient settings with compassion and clinical insight. Experienced in driving service improvements through policy and procedure development, while also committed to the growth of others through supervision, mentoring, and education of staff and students. Organised and proactive, with excellent time management skills, strong digital literacy, and the ability to communicate clearly and effectively across diverse audiences. Why Work at St Vincent’s?
