Permanent Position - 3 days per week The Inpatient Therapy Program offers specific group and individual evidence based therapy to patients across General Acute Adult Psychiatry, Substance Abuse & Addiction and Aged Psychiatry programs. We are seeking a highly motivated and experienced (5 years minimum) Social Worker with NDIS, My Aged Care and Mental Health experience to work across all programs. This role is responsible for providing effective, high quality 1-1 social work and welfare service delivery to assist our patients, their families and carers. Experience and interest in providing group therapy is not essential but desirable. What we are looking for: Liaise with external stakeholders to facilitate services and/or accommodation through NDIS or My Aged Care Systems Assist with patient centred care, treatment, risk and capacity assessments, education and robust discharge planning. Provide effective and relevant welfare service assistance for patients, families and carers using clinical and professional interventions Actioning referrals to community services, NDIS and My Aged care supports Conducting patient and carer consultations, family meetings as required Contribute to the development, implementation, delivery and evaluation of group programs Broaden current service activities Liaise with stakeholders to oversee the efficient, high quality delivery of Social Work across the Hospital Skills and Experience: Approved degree in Social Work and AASW registration Minimum 5 years experience working in a hospital, Aged Care or Community setting Excellent oral and written communication, interpersonal skills and ability to build positive relationships with all stakeholders Ability to problem solve, manage, improve and make decisions in the planning and allocation of resources to ensure optimal outcomes Proven multi tasking, problem solving and time management skills Highly professional with an empathetic, respectful and approachable manner Ability to work collaboratively as part of a multi disciplinary team Renumeration: Salary will be commensurate with qualifications and experience and as per the relevant Delmont EA.
