About the role: Reporting to the Senior Welfare/Placement Officer this role, some of your duties for the role will include: Working across FMC & NHS, including some weekend work Referral management, including conducting placement meetings with patients and families as they transition to Residential Aged Care Facilities from hospital. Assisting patients and families with information about practical assistance such as access to transport and accommodation. Supporting patient with practical welfare support where appropriate. Contributing to coordination, evaluation and implementation of service improvement programs and departmental planning Administrative tasks directly related to a patient intervention or episode of care (as distinct from administration specific roles) About you: Knowledge of the Aged Care Sector very desirable Ability to relate to and empathise with a wide range of people, including patients, families & carers, clerical staff, clinical staff, Aged Care Assessment Teams, Directors, Supervisors and Managers of Residential Aged Care Facilities and Supported Residential Accommodation facilities Demonstrated ability to exercise adaptability, flexibility and advocacy skills when dealing with patients, families & carers, working with distressed, confused, abusive and sometimes violent patients and families Experience in using a range of computer software packages, e.g. spreadsheets, word processing and database packages Please see the attached Role Description for additional information.
