The medical team include three palliative medicine physicians/ specialists, a geriatrician with palliative medicine interest and two advanced trainees. The team works closely with the unit multidisciplinary members and referring physicians/specialists in catering for symptom management, coordination of care, complex discharge planning and end of life care management. The Role The Consultant in Palliative Medicine/ Palliative Medicine Physician leads and manages the delivery of evidenced based Palliative medicine to patients and families across Cabrini. The Palliative Care service operates in an integrated model across 4 service arms: consult; inpatient, ambulatory and community. The doctor will oversee the clinical service delivery to patients and families as directed by the Director of Palliative Medicine. About you Cabrini’s Palliative Care Consultants provide compassionate and competent care to all patients, working within the Mission and Values of Cabrini Health.
