Position Overview: We are urgently seeking a compassionate and experienced Male Disability Support Worker with a strong background in Disability support. This is a unique opportunity to work closely with clients to build life skills, access community services, and help them achieve their personal goals. Key Responsibilities: • Develop and implement personalized support plans to meet individual client needs. • Provide assistance with daily living skills, community engagement, and education/employment support. • Offer mental health support and guide clients toward greater self-sufficiency. • Assist with social activities, transport, personal care, and other needs as required. • Maintain accurate documentation and case notes to ensure quality service delivery.
