The focus is on psychosocial assessments, risk assessments, complex discharge planning and family support. About Us We are a busy and dynamic team assisting patients and families by providing a professional service for patients referred across the acute and sub-acute inpatient setting. Our Social Work service includes Acute, Sub-Acute (Aged and Rehabilitation) and specialty areas such as Intensive Care, Emergency Department and Women’s and Children’s services. The successful applicants will have the opportunity to accept available shifts and some short term contracts across all sites as part of a busy, energetic and friendly Allied Health team. These positions may be based at our hospitals; Casey, Clayton, Dandenong, Kingston, Victorian Heart Hospital and Moorabbin sites. About You Qualified and Experienced: You have a minimum of 4 years post-graduate experience with a strong focus on patient-centered care. Grade 1 Social Workers may also be considered based on experience. Adaptable and Enthusiastic: Your hard-working nature allows you to thrive in changing settings and shifts, including weekdays, weekends, and public holidays. Dynamic Professional: You are passionate about contributing positively to the health and well-being of your patients.
