Position Details Position Title: Social Worker Classification: SC21 (Grade 2 Year 1) Salary per hour: $44.77 superannuation generous salary packaging options Employment Type: Ongoing -Up to full time available will consider Part Time applications Award: Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise agreement 2021-2026 Role description Dhelkaya Health are seeking a Grade 2 Social Worker permanent ongoing to support leave backfill for our Continuing Care Social Workers . The Social Worker will be responsible for delivering person-centred Social Work services across all areas of Dhelkaya Health, (Inpatient Units, TCP, Ambulatory Care / Outpatients and Complex Care, Community Health, and Residential Aged Care) including the Castlemaine and Maldon campuses and provision of care in client's homes in the community. Social Workers are an integral part of our multi-disciplinary team working collaboratively to provide a responsive, integrated and flexible approach to service provision. Social Work supports a client centred approach to support people with chronic and complex illness to achieve optimal health outcomes in their local communities. The Social Worker will work in close collaboration with nursing, medical and allied health staff to deliver the best outcomes for clients, their family and carers, while also liaising closely with other service providers both within the hospital and the community setting. This Social Work position may be redirected to work in other areas of the organisation to ensure adequate Social Work cover across all areas at all times and to support flow and continuity of care, dependent on business requirements About you Bachelor/Masters of Social Work degree and eligibility for AASW membership High level communication and interpersonal skills, with the ability to establish positive relationships with patients and colleagues. Demonstrates an understanding and experience in the application of the Social Model of Health framework in the areas of chronic disease and health self-management, aging and cognitive, intellectual or physical disability. Experience working across a range of SW clinical areas in health care environment, particularly with older people.
