This position is full time ongoing, working from Monash Medical Centre in Clayton, where you will play a vital role in delivering high-quality, evidence-based psychosocial care to patients and families across a broad range of clinical areas. This position requires flexibility to respond to clinical demands, working collaboratively within multidisciplinary teams across Monash Health campuses. Your responsibilities will include psychosocial assessment, counselling, advocacy, discharge planning, and connecting patients to appropriate community resources. You will work with individuals experiencing significant social complexity, including those affected by family violence, elder abuse, and other vulnerabilities. A sound understanding of systems such as My Aged Care, the NDIS, and legislative frameworks relevant to vulnerable populations is essential.This role involves crisis intervention, complex discharge coordination, support in bereavement and perinatal loss, and planning for residential aged care. You will also contribute to the service’s education and development efforts, including supervising undergraduate social work students. The position operates during standard business hours, Monday to Friday, with participation in the on-call and weekend roster required at acute sites. You may be required to work at any Monash Health location as needed, either temporarily or permanently, and will be supported in developing expertise across a range of clinical settings.
