See more at our website - www.atsichsbrisbane.org.au Job Description Please note that it is a genuine requirement that this position be filled by an Aboriginal or Torres Strait Islander person in accordance with s25 of the Anti-discrimination Act 1991 (Qld) About The Role As a key member of the ATSICHS Brisbane team, the Care Connect Worker Effectively engage with and support clients experiencing social and emotional wellbeing issues who present to the Logan Social Health and Primary Health Clinic. The role will support the needs of families and individuals by operating within a strengths and priority-based framework. The Care Connect Worker will deliver cultural support for clients, providing a range of social and wellbeing assistance to clients in meeting their holistic needs. Supports will be goal driven and focussed on building the resiliency and capacity of individuals. Duties Provision of Social Emotional Wellbeing Support. Assist clients to address practical issues relating to SEWB needs. Support to coordinate care. Management of data. Commit to a high level of quality management for ATSICHS through use of LogiQC and MOBnet quality management systems. Cultivate productive working relationships at all levels, which support the nurturing of internal and external relationships, facilitates cooperation, trust and respect, values differences and diversity and guides, mentors and focuses on the development of staff. Desired Skills and Experience Required: Minimum Certificate III in Community Services, Mental Health or Disability, or three years of equivalent professional experience. Demonstrated ability to work with Aboriginal and Torres Strait Islander communities and their leaders, respecting traditional culture, values and ways of doing business. Demonstrated experience in facilitating positive behaviour change with individuals and families. Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team. Demonstrated IT capability - experience in computer-based data management systems and Microsoft Office. A willingness to provide supports and engagement to Clients outside of a clinic environment. Working with Children’s Card (Blue Card) and NDIS Worker Screening (Yellow Card) or be eligible to apply. Current C Class Drivers Licence (Qld) and the ability to travel daily around SEQ. Preferred Previous experience in a multi-disciplinary team environment.
