Medical Social Worker — Canberra Region, Australian Capital Territory

Summary: We are seeking a compassionate and experienced Medical Social Worker to join our team. The successful candidate will be responsible for providing psychosocial support to patients and their families, helping them cope with chronic, acute, or terminal illnesses. The Medical Social Worker will collaborate with healthcare professionals to develop care plans, connect patients with community resources, and advocate for patient needs. Responsibilities: Conduct psychosocial assessments to determine the social, emotional, and environmental needs of patients and their families. Develop and implement individualized care plans based on assessment findings. Provide counseling and support to patients and families to help them cope with illness, hospitalization, and treatment. Assist patients in understanding their diagnosis, treatment options, and the implications for their daily lives. Connect patients and families with community resources, such as support groups, financial assistance, and home care services. Advocate for patients’ needs within the healthcare system and with external agencies. Collaborate with healthcare professionals, including doctors, nurses, and therapists, to ensure comprehensive care. Facilitate communication between patients, families, and healthcare providers. Assist with discharge planning, ensuring a smooth transition from hospital to home or other care settings. Maintain accurate and timely documentation of assessments, care plans, interventions, and outcomes. Participate in multidisciplinary team meetings to discuss patient care and coordinate services. Stay up-to-date with current social work practices, healthcare regulations, and community resources. Qualifications: Bachelor’s degree in Social Work (BSW); Master’s degree in Social Work (MSW) preferred. State licensure or certification as a Social Worker (e.g., LCSW, LMSW). Proven experience in medical or clinical social work. Strong knowledge of social work principles, healthcare systems, and community resources. Excellent communication, counseling, and interpersonal skills. Ability to work effectively as part of a multidisciplinary team. Strong organizational and time-management skills. Compassionate and empathetic approach to patient care. Ability to handle emotionally challenging situations with sensitivity and professionalism.
