They work under general guidance from the person seeking support and their family, following defined guidelines. Skills: - 1 or more years of experience in a similar role - Comprehensive knowledge of NDIS support items under NDIS Guidelines - Strong communication and interpersonal skills - Ability to work effectively with individuals with disabilities - Advocacy skills to help clients access community services Responsibilities: - Provide support to patients with disabilities in daily living activities, community engagement, and empowerment efforts - Adhere to Fellowship policies and procedures while assessing and supporting clients - Establish positive relationships with community agencies, clinical services, and other providers for effective communication and care coordination - Assist clients individually or in groups to set goals, make decisions, overcome barriers, and achieve personal objectives - Advocate for clients' access to necessary services within the community - Monitor participant referrals, progress against NDIS Goal Plans, data collection,and reporting as needed Qualifications: - 1 or more years of experience as a Disability Support Worker or similar role - Certificate III in Individual Support (Disability) or equivalent qualification preferred - Valid First Aid certification - National Police Check clearance
