Challenging and rewarding work environment Friendly and dynamic team About the Role Your role will be to provide high quality Social Work assessment and management for clients referred to Polio Services Victoria, many of whom come from diverse cultural backgrounds. Additionally, your role will include: Collaborative work with the multidisciplinary team members, referring professionals, community professionals and other key stakeholders within a flexible, multidisciplinary structure. Provide ongoing education and advice to clients and health professionals regarding psychosocial management of people with a history of contracting polio. Duties Provide clinical leadership in Social Work practice for PSV Drive best practice based on evidence and demonstrated outcomes Delivery of accountable, client focused, high quality social work services - assessment, care planning, referral management, reviews, counselling and carer support to all clients within your caseload Delivery of groups with clients in collaboration with team members.
