About you: In your new role you will: Assess the psychosocial needs of patients, families and caregivers to identify clinical risk. Translate information from clinical assessments and diagnostic conclusions into appropriate social work interventions. Provide advocacy to address client needs and ensure equity and access to supports and services, including the Public Guardian, Queensland Civil and Administrative Tribunal, aged care residential and home care providers, NDIS providers, and other agencies to benefit patients and families. Collaborate in cross cultural liaison and consultation frameworks with government and non-government service providers to sustain and improve health outcomes for patients. Provide clinical support and education to less experienced staff, students and multidisciplinary team members, commensurate with experience.
