Company Description The Home Care Package ( HCP ) team provide individually planned and coordinated suite of support and services designed to assist older people who need support to remain living independently, in their own homes. Job Description Multiple casual positions available (ideally we are looking for candidates who can work a minimum of 15hrs per week) Located across the Eastern, Outer Eastern and Yarra Valley regions. Office based in Ferntree Gully. Facilitating Independence: Help clients with personal care and transportation to appointments, ensuring they stay connected and active in their communities. Who we are looking for If you have experience working as a Support Worker, are qualified with a Minimum Cert III in Aged Care, Community Support or related field, and enjoy variety in your day, you might be exactly the person we're looking for Minimum Cert III Qualification in Aged Care, or Individual Support (must have completed aged care components). Experience working with older adults. The ability to follow and implement individual service plans, healthcare, and positive behaviour support plans. A desire to use your skills and experience creating fun and interesting activities that support older adults to engage more consistently with their community and peers. A valid driver’s licence. Qualifications Minimum Cert III Qualification in Aged Care, or Individual Support (must have completed aged care components).
