Southern Adelaide Local Health Network – Flinders Medical Centre – Rehabilitation, Aged Care and Palliative Care Salary $104,180.00- $111,746.00 AHP3 (Pro Rata) | Part Time 19 hours per week – Contract Until May 2025 Southern Adelaide Local Health Network is seeking passionate, talented, and caring Senior Social Worker to join the Rehabilitation, Aged Care and Palliative Care team The Geriatric Evaluation and Management (GEM)Home service at SALHN functions with a ‘home first’ principle by providing sub-acute hospital level care, goal directed therapy, and support to older people in the comfort of their own environment. The Senior Social Worker participates as a member of the GEMHOME team, which provides care to older people with complex medical, functional and/or psychosocial needs in their own home in lieu of an extended or avoidable hospital admission. About the role: Reporting to the Advanced Nurse Unit Manager, and Director of Social Worker RAP, as the Senior Social Worker duties for the role may include: Providing care planning, case management and liaison to ensure sustainable social support in place in community Documenting assessment findings and interventions in accordance with service standards Linking clients and carers with relevant community resources and services Collaborating with other team members to initiate, develop, implement, and evaluate therapy/activity groups About you: Ability to work under pressure, prioritise workloads and meet deadline Demonstrated ability to provide social work clinical services as a member of an interdisciplinary team A degree or equivalent in social work, entitling membership of the Australian Association of Social Workers Experience providing care planning, liaison and case management Experience as a social worker in a health setting Please see the attached Role Description for additional information.
