Social work duties include, psychosocial assessment and provision of social work intervention including, assistance with financial and accommodation issues, adjustment to diagnosis, advance care planning, referral to community agencies, carer support, and family violence risk assessment and intervention. Two casual positions are available: Flexible casual bank: variable hours, variable roster (immediate start) Seeking social worker available for casual relief/ leave cover in our community palliative care, inpatient unit and our outpatient clinic for people with neurodegenerative diseases. Experience in palliative care, health or aged care will be considered favourably. Any shifts will be between Mon-Friday What you bring Experience in hospital social work, palliative care, progressive neurology and/or aged care Strong knowledge of systems and supports that help people live well with advanced illness and/or aging Experience in working with the MyAgedCare system, including the MAC provider portal and DEX reporting portal Ability to work autonomously and as part of a team Tertiary qualification in social work Eligible for registration with the Australian Association of Social Work (AASW) A desire to join a team committed to the care of others in a mission based organisation Please submit a resume and cover letter. Calvary Bethlehem is a public health care service that specialises in palliative care and progressive neurology through our 32 bed inpatient unit, Level 5 Statewide Progressive Neurological Diseases (SPNDS) outpatient clinic, home-based Community Palliative Care Service (CPCS) and Day Centre. Calvary Bethlehem is also a National Disability Insurance Agency (NDIS) and Commonwealth Home Support Program (CHSP) provider. Vaccination against seasonal Influenza is a mandatory requirement for healthcare workers in Victoria, and as such, all employees at Calvary Kooyong must provide evidence of mandatory vaccinations upon application. All employees must be willing and able to wear Personal Protective Equipment when required, which at a minimum is a surgical mask. Additional PPE requirements are role-dependant.
