We believe our people make the difference. About the role We are seeking a Social Worker to join our Social Work team in General Medicine at The Box Hill Hosptial. You will have relevant hospital experience and be able to work within a strong multidisciplinary team. To be successful, you will need to have a good understanding of social work theories and apply them to hospital practice. It will be essential to have a sound understanding of complex discharge planning within an acute hospital setting in particular an understanding of Transition Care Program, My Aged Care and the NDIS. The successful candidate will need excellent communication skills to collaborate with patients, families, other disciplines, and community stakeholders. This is a fast-paced environment and requires timely and quality discharge planning and the successful candidate will need to work flexibly, creatively and collaboratively. About you To be successful in this role you will possess: • Holds a degree qualification in Social Work that has been approved by the Australian Association of Social Workers (AASW) for membership as a practicing Social Worker. • Maintains compliance with the AASW Continuing Professional Development Policy in order to meet the Eastern Health Allied Health credentialing standard. • Demonstrated capacity to work effectively in a fast-paced health environment • Excellent problem-solving skills • Demonstrated capacity to work effectively in a multi-disciplinary team • Excellent communication skills • Basic knowledge of the relevant legislation and frameworks Vaccination against infectious disease is a mandatory requirement of this role. An offer of employment is conditional on you providing evidence that you are currently vaccinated against COVID-19 with booster & Influenza, prior to commencing employment.
