Position Overview The Support Worker – Complex Care plays a crucial role in providing specialised, responsive, and person centered support to individuals with complex and high-level psychosocial needs to achieve their goals, and enhance their physical and mental health to ultimately, improve their quality of life. In this role, the Support Worker – Complex Care is responsible for delivering comprehensive support that includes but is not limited to personal assistance, behaviour management, therapeutic support, and community engagement. You will be dedicated to working closely with our most complex residents. Currently, this includes providing tailored support to a variety of our complex residents, ensuring that their unique needs are met with the utmost care and dedication. The role requires advanced skills in managing complex needs, crisis intervention, and implementing individualised care plans prepared by wider clinical teams. Key Responsibilities: Provide comprehensive support, including personal assistance, behaviour management, therapeutic support, and community engagement. Follow and implement tailored Behaviour Support Plans to address complex behaviours. Apply de-escalation techniques and evidence-based behavioural interventions. Maintain current and accurate documentation of participant care and behaviour. Act as a liaison and advocate for participants within the community. Qualifications and Experience: Certificate IV in Disability Support (or equivalent) is required. University qualification is desirable. Preferably 2 years' experience in supporting individuals with complex needs. Strong understanding of behavioural theories and practices. Excellent communication, interpersonal, and written skills. Demonstrated empathy, patience, and resilience.
