Job Description In this role, as a Complex Case Manager Resident, you will be part of a dedicated team of case managers, providing guidance, training, and supervision to ensure effective service delivery. You will collaborate with multidisciplinary teams and community partners to develop and implement personalised care plans that address the unique challenges faced by clients. This role requires strong organizational skills, empathy, and a deep understanding of the healthcare challenges faced by individual. Key responsibilities Manages patient files, ensures compliance with policies, handles legal documentation, and communicates with external agencies. Ensures that policies and procedures are being followed, including safety protocols, guidelines, and facility regulations. Coordinates communication between different departments and external agencies. May supervise administrative staff and ensure operational efficiency in terms of case handling, scheduling, and facility management. Responsible for data collection and reporting, tracking patient statuses, and creating reports for internal or external review.
