About the role We are currently seeking a Social Worker for a 3-month locum contract based in Adelaide. Key responsibilities include: Providing assessment and diagnosis of emotional and social health issues, including complex/chronic co-morbidities and high need short term therapeutic programs to improve self-efficacy, independence, access to community services and psychosocial outcomes (including quality of life) for the client group. Responsible for case management, facilitating discharge planning, and delivering social work therapeutic interventions as needed. Working closely with the Lifestyle Coordinator to ensure holistic opportunities for group-based models. Collaborating and communicating effectively with the multidisciplinary team in planning and provision of high-quality social work services and participates in discipline and clinical service education, quality improvement and planning activities. To be successful in the role you will have: Bachelor of Social Work (or equivalent) Membership with the Australian Association of Social Workers Experience with SACAT essential Australian working rights Australian drivers' licence National Police Clearance Aged/Vulnerable NDIS Worker Check Working with Children Check and Aged Care Check Min 3 years' experience in the field Why apply through Healthcare Australia?
