The Social Work Service in North West Tasmania supports the inpatient discharge process at both hospitals and the outpatient and community team provides hospital avoidance and wellness and reablement support to clients through Primary Health funded services and Commonwealth Home Support Services. Social Work also offers support to: Vulnerable adults who have experienced an exacerbation of a chronic health condition and have been discharged from the hospital's Emergency Departments and require short-term Social Work intervention to avoid hospital re-presentation. Community-based, health related groupwork programs Adults (and their Carers) recently diagnosed or experiencing significant health changes and require support with adjusting and transition The role: As a member of the Primary Health Services North West Social Work Services team, the incumbent will contribute to the development, provision and evaluation of Social Work Services for individual clients and client groups and/or the community, in accordance with the Code of Ethics of the Australian Association of Social Workers and the Code of Conduct of the Department of Health.
