SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it The Cardiac Rehabilitation (CR) service at Sutherland Hospital provides a recovery program for cardiac patients recently discharged from hospital following a Myocardial Infarction, revascularisation, or heart surgery etc. The CR staff liaise closely with cardiology staff across Yarrabee, the Coronary Care Unit and the Sutherland Heart Clinic for patient referrals and ongoing staff education. The CR team consists of a Clinical Nurse Consultant, a Clinical Nurse Specialist, an Exercise Physiologist, and an Administration 0fficer. This close team strives to maximise each patient’s recovery with support, education, and exercise. The program offers patients individualised assessments and exercise prescription as well as group education and ongoing support intervention via virtual telehealth service or in person gym classes. Both telehealth and face to face are offered after consultation and in regard to their goals and requirements. The weekly supervised gym program provides a safe environment to help our patients with confidence to be able to exercise independently and awareness of heart health. The Registered Nurse coordinates and delivers high quality nursing care to patients/clients of The Sutherland Hospital consistent with SESLHD and The Sutherland Hospital policies, procedures and standards. Our Hospital and Community: Established in 1958, The Sutherland Hospital campus offers a comprehensive range of inpatient and community services to around 226,000 residents of the Sutherland Shire, colloquially known as ‘The Shire’. Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others. Demonstrated ability to proactively engage with patients/clients to enhance service delivery and to guide others in providing service excellence. Demonstrated ability to work as part of a multi-disciplinary team and apply professional and ethical boundaries when dealing with complex situations. Basic computer skills and proven ability to use health information systems. Detailed knowledge and understanding of risk management, safety and quality principles and the role of the Registered Nurse in applying these principles. Demonstrated recent post registration experience in Cardiac Rehabilitation and/or Cardiology within an acute care setting, including experience in providing Cardiology nursing assessment, education and intervention.
