If this sounds like you, get in touch today About the role: As a Community Support Worker , you will play a vital role in assessing client needs, providing personalized care, and helping individuals achieve their goals. Your key responsibilities will include: Assessing the needs and strengths of clients to develop customized support plans tailored to their abilities. Collaborating with families and professionals to design and implement individualized support plans. Conducting home visits and assessments to gain insight into clients' environments and identify areas where additional support may be needed. Facilitating access to essential community resources such as healthcare, education, and housing services. Organizing and leading group activities and workshops focused on enhancing family relationships and building resilience. Providing direct support and practical assistance, including counselling, advocacy, and emotional support. Keeping detailed and accurate records of all client interactions, progress, and outcomes to ensure continuity of care. Working closely with team members and external agencies to provide a holistic approach to client support. Participating in ongoing training and professional development to maintain a high standard of care and stay current with best practices. This role requires a compassionate, proactive approach and the ability to foster meaningful relationships with clients and their families. Requirements: A relevant qualification. At least 2 years of previous experience in Disability Support or Community Support Worker. A valid full driver’s license and access to your own vehicle. Comprehensive Car Insurance. NDIS Workers Screening Check (Service NSW). A current First Aid & CPR Certificate. Training or experience in manual handling, bowel care, and managing challenging behaviours is highly desirable.
