Job location Thomas Embling Hospital,located in Fairfield, just 15 minutes north of the Melbourne city centre.Due to our announced expansion, we have a need for experienced like-minded professionals to join our team. Thomas Embling Hospital is a secure mental health hospital, currently 134 bed, hospital providing acute care, rehabilitation, and continuing care programs. With planned expansion commencing 2024 with significant increase in access to care, we will have a dedicated Women's service and an additional medium secure men's programs. Now is the time to join us About the Role: The Social Worker (Grade 2) is responsible for providing social work services in a custodial care environment. The involves unit-based interdisciplinary team work and providing tailored individual and group interventions, family/carer engagement, advocacy, engaging external supports and recovery/psychosocial support activities. Duties: Work as part of a interdisciplinary team and hold a Grade 2 clinical Social Worker role as part of the Forensicare Social Work Service. Provide direct clinical practice and consultation with teams providing recovery-oriented custodial mental health care. Develop and implement social work interventions focusing on the needs of individual, families/carers and consumer groups. Develop linkages with community-based services, such psychosocial disability providers, transition supports, legal representatives, courts and other external stakeholders. About You: Successful candidates will have experience or transferable skills in mental health care and/or secure, forensic or correctional settings. Demonstrate family violence risk assessment and management activities aligned to the Multi-Agency Risk Assessment and Management (MARAM) Framework, the Family Violence Information Sharing Scheme (FVISS) and Child Information Sharing Scheme (CISS) legislative requirements. Thrive in an interdisciplinary practice environment and undertake social work assessments and interventions aimed at addressing risk reduction and psychosocial needs. Proven skills and experience in developing and implementing complex case plans involving direct and indirect social work practice. Demonstrated ability to liaise effectively with various external agencies and organisations.
