: Social Worker - Grade 2 Outstanding opportunity for an Oncology Social Worker Friendly and dynamic team Full time position based at St Vincent's Hospital Melbourne About the Role An opportunity currently exists for an experienced Grade 2 Social Worker to join our team on a permanent basis. At St Vincent's we have an approach to working with patients which puts them at the heart of their care. This means that Social Workers in the Cancer and Chronic Illness Team are able to build rapport and a professional relationship with patients across their hospital journey and become specialists in their clinical area of focus. This position will work with patients in a range of settings including the cancer centre, outpatient clinics and inpatient acute and oncology wards. To fulfil this role, it is essential that you have experience working in an inpatient hospital social work position. Experience in a hospital setting (preferably acute) would be highly regarded. You will be required to: Use your skills to undertake multi-dimensional, comprehensive psychosocial assessments, apply clinical reasoning, and formulate appropriate complex intervention and discharge plans Liaise with patients, their families and/or carers to co-ordinate, facilitate and communicate patient treatment and outcomes Actively participate in quality improvement and research activities Provide supervision and participate in clinical education programs as required Promote and develop positive relationships within the department, wider organisation and with community partners Classifications for this position will range from SC21 - SC24 ($88,467 - $101,691 per annum).
