About you: You are a qualified Social Worker with demonstrated high-level knowledge, expertise, and skill in Social Work assessment, interventions, counselling, and bereavement support for clients with complex psycho-social needs in a Palliative Care setting. You are confident to provide clinical advice to professional and operational supervisors regarding Social Work service delivery and improvement opportunities for complex care within the inpatient and community Palliative Care Setting can provide Social Work clinical services of a complex nature, exercise risk management and apply established ethical principles and practice standards in complex psychosocial situations. Your opportunity: Deliver high level clinical Social Work services for community clients of the Community Palliative Care Service to ensure optimal assessment, clinical intervention, discharge planning, end of life care and bereavement support as part of a multidisciplinary team providing specialised palliative care. This position may provide clinical Social Work services for Palliative Inpatients as required, to continue service delivery during periods of leave. Provide leadership within the CHHHS Social Work Professional group, including clinical practice supervision, assistance with performance management processes and participation in the business and quality activities to ensure timely and effective Social Work service delivery in the Palliative Care setting. Your role: Deliver high level social work clinical services in accordance with the Australian Association of Social Workers (AASW) Code of Ethics and AASW professional practice standards using highly developed assessment, intervention and problem-solving skills which reflect evidenced based practice, initiative and innovation and are undertaken with a high degree of independence. Exercise independent clinical judgement and more complex decision making in areas such as psychosocial assessments; counselling; crisis intervention and bereavement support in both inpatient and community settings for patients with a life limiting illness and including families and carers. Understanding and application of theory to practice of current theories of grief and loss. Apply high level professional judgement and decision making to manage a complex caseload, adapting routine and non-routine clinical practice based on valid and reliable clinical evidence with minimal direct clinical practice supervision. Provide high level advocacy to address patient needs and ensure equity and access to services.
