About the Role An exciting opportunity exists for a Senior Social Worker to join a dynamic Allied Health team at the Mental Health Intensive Recovery Program (Bed Based Units). The Secure Extended Care Unit (SECU) provides secure, longer-term care for people who have unremitting and severe symptoms of mental illness or disorder and are unable to be safely or adequately treated in less restrictive settings. Consumers, their families and supporters, are provided with specialised mental health treatment, therapies, and programs tailored to support recovery, and minimise the ill effects of long-term care. The Senior Social Worker is an integral member of the multidisciplinary team, providing evidence-based care in collaboration with the consumer, their carers, families, and supporters. The Senior Social Worker works with a Recovery Oriented framework to deliver high quality Social Work services to consumers, including therapeutic interventions, and collaborating in formulating and facilitating complex discharge planning. The Senior Social Worker works assertively to strengthen the discharge planning process, demonstrating the ability to work in a dynamic environment to meet the competing needs of our consumers, families, and their supporters. For the right candidate, part time will be considered. Key Responsibilities Guide, deliver, and support the provision of specialist psycho-social assessment and systemic analysis. Guide, deliver, and support provision of care planning for consumers with complex psychosocial needs in collaboration with the consumer and their carer/s, the multidisciplinary team, and other treating health professionals. Provide consultation around targeted psychosocial interventions to support families and carers of our consumer group. In collaboration with the multidisciplinary team and Lived and Living Experience workforce, monitor and facilitate the progress of consumers through the course of their inpatient stay from admission to discharge with a specific focus on maintaining and establishing robust supports in the community which will enhance recovery and help prevent relapse and avoid potential readmission. Skills & Experience Clinical experience of at least 5 years post Qualification. Clinical expertise including in the provision of biopsychosocial assessment, formulation, care co-ordination, evidence-based systemic interventions, and recovery-oriented practice. Demonstrated ability to provide discipline specific supervision, and support Social Work activities. Knowledge and ability to analyse and conduct quality improvement activities. Act as a resource and provide Social Work expertise and education to staff, demonstrating a strong current theoretical knowledge and evidence base for Social Work practice in mental health.
