With a booming, culturally rich and diverse community, and less than an hour’s drive from the CBD, Melbourne’s North is fast becoming the place to be  About Kilmore District Hospital: Kilmore District Hospital (KDH) is located in Victoria in the Mitchell Shire and services a population over 35,000 that extends to Broadford and Pyalong in the north, Wallan and Craigieburn in the sourth, Lancefield and Romsey to the West, and Whittlesea to the east. Dianella Hostel provides residential and respite care for up to 30 residents based on the principles of ageing in place. Dianella Hostel provides care for residents with varying levels of dependence and is supported by a staff mix of Registered and Enrolled Nurses, Health Care Workers, Volunteers and Lifestyle Coordinators. The Role: Dianella Hostel at Kilmore District Health has multiple Health Care Worker positions available - Permanent Part Time and Fixed Term Part Time Hours. This role requires a highly motivated Health Care Worker (HCW) who is an excellent communicator and strives for best practice. The HCW advocates for the rights of residents and is a role model for professional and respectful behaviour. Reporting to the Nurse Unit Manager, the HCW role is responsible for supporting the coordination and delivery of the complex needs of our resident group to ensure that residents and their families receive optimal access to the appropriate multidisciplinary health team and support services within the community. The EN is accountable for the promotion of a positive organisational culture that supports the Residential Aged Care Nurse Unit Manager to communicate and implement organisational change. The HCW position is a pivotal role within the residential care facility ensuring residents receive optimal care, quality of life and a safe living environment. The role is responsible for ensuring the achievement of excellence in care delivery in line with organisational policies & procedures, best practice and legislative requirements. The HCWis also responsible for ensuring organisational outcomes are in line with departmental and organisational strategic objectives, is a highly motivated and an excellent communicator striving for best nursing practice. To be successful in this role you will have: Probity and clearance required prior to commencement including current Victorian Driver’s Licence, Police and Working with Children Checks Experience working in Aged Care Ability to work within a flexible roster system.
