ABOUT THE ROLE The Social Worker will provide social work services to clients, their families and representatives for the Transition Care program and ensure practices are aligned with Amana Living’s core values. They help facilitate the appropriate transfer of residents and clients between both their home and Residential Care facilities. The Social Worker plays an integral part in ensuring the psychosocial safety and wellbeing of clients and their representatives is upheld whilst they access Amana Living Transition Care services. This is a permanent part-time role offering 30.40 hrs per fortnight. Week One: Tuesday: 0800-1615 Thursday: 0800-1615 Week Two: Tuesday: 0800-1615 Thursday: 0800-1615 Key responsibilities: Undertaking social work assessments, including psycho social assessments for all assigned residents and clients in consultation with other team members. Planning and undertaking social work interventions with residents and clients, in conjunction with their families. Providing care coordination, counselling, advocacy, mediation, referrals and support for residents and clients when transferring them to either their home environment or a Residential Care facility. Engaging in collaborative practices to achieve positive outcomes for residents/clients and their families, including psycho social factors. Providing consultation and support to Nursing and Allied Health professionals regarding psycho social aspects of care for residents and clients. Reviewing treatment objectives and goal oriented discharge plans to ensure their continued application and improvement where necessary. Recording resident and client treatment results and disseminates this information to all relevant personnel. Evaluating the effectiveness of the social work programs in place for residents and clients and ensuring they continue to meet their needs. Participation in regular multidisciplinary team meetings Effectively communicating with personnel both inside and outside of the organisation Ensuring compliance is maintained in line with Amana Living’s policies and procedures and accreditation standards are sustained. Attending and actively participating in compulsory training sessions and meetings. Contributing to the continuous improvement of own work practices. ABOUT YOU: The successful candidate will have proven exceptional customer service experience, being responsible for identifying the service needs required and best match the enquiry to the appropriate Amana Living service, ensuring the service delivery is focussed on the needs of the client. Effective planning and organising skills Essential Criteria National Police Clearance Certificate (no older than 6 months) Successful completion of pre-medical form and reference checks. Successful completion of the key physical requirements assessment. Bachelor of Social Work, current registration with Australian Association of Social Workers. ‘C’ Class drivers licence. Demonstrated knowledge of current issues relating to social work. Demonstrated ability to utilise a variety of social work interventions. Well developed verbal, written and interpersonal communication skills.
