Social Worker Bed Based Services Full and Part Time Grade 2 Social Worker Multiple roles available: Box Hill Hospital (Acute) and the Peter James Centre (Sub-acute) Support the access and flow through acute and sub-acute services. About the role The social work roles are based at Eastern Health’s largest acute and sub-acute sites. The social worker will be allocated a ward, however will be required to work in a flex capacity on wards where the regular social worker is on leave. The Grade 2 Social Worker will provide leadership and ensure high level, evidence-based Social Work services to patients. The worker will need to demonstrate flexibility in order to provide an efficient response to referrals. Social Worker will work closely alongside Allied Health colleagues and the rest of the multi-disciplinary team. About you To be successful in this role you will possess: Eligibility for membership of the AASW Evidence of current CPE, as per AASW requirements Highly developed interpersonal skills A strong commitment to high quality care The ability to work effectively as part of a diverse team and also work autonomously as required Excellent communication and organisational skills Excellent risk assessment skills The ability to lead complex discharge planning Conflict resolution skills Please refer to the attached position description for further details of the role.
