About the Role The Social Work Renal program is looking for an experienced Grade 2 Social Worker to join the Renal team that operates across Casey, Clayton, Cranbourne, Dandenong, Moorabbin and the Kingston Centre. This is a part-time (45.60 hours per fortnight or 0.6 EFT), permanent role primarily based at Casey Hospital and the Cranbourne Centre. The Social Work Service at Monash Health assists patients and families with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning. Social Workers function as members of the multidisciplinary team and have a pivotal role where social complexity exists. The Social Work Service operating hours are standard business hours Monday to Friday. A weekend service is provided by roster at Casey, Clayton, Dandenong and Victorian Heart Hospital sites and an on-call service is also provided to the acute sites. This role provides a clinical service to the Dialysis Units. Experience working with individuals in a healthcare setting with a chronic illness is desirable. This clinical role provides an opportunity to showcase your skills in working with people from a range of diverse backgrounds as well as completing complex psycho-social assessments and development of patient centred care plans. It is a requirement of this role to participate in the Emergency Department and on-call rosters. A Day in the Life As the Grade 2 Social Worker, you will be an experienced and skilled clinician with consolidated psychosocial assessment and planning skills and use evidence to inform practice to enable you to carry a complex clinical caseload. You will also be required to provide team leadership, lead Quality and Improvement initiatives, and provide education and training to staff and students. Training of undergraduate Social Work students and secondary school students is undertaken throughout the year. A caseload will be carried in designated areas, maintaining clinical expertise. Having a sound understanding, and working knowledge of the range of the service systems including Family Violence, Child Protection, My Aged Care, Transition Care Program, and the National Disability Insurance Scheme (NDIS), Mental Health and the recovery model is essential, to ensure we provide high quality, evidence based social work service to all patients and families at Monash Health. It will also be expected that you will have responsibilities and actively participate in quality and research activities. You will work in accordance with and is professionally accountable to the standards of the AASW and Social Work Credentialing and Scope of Practice, Supervision, Code of Ethics and Practice Standards. The position works closely with the internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community based setting. You will have demonstrated experience as a social worker who is able to work collaboratively with the broader multidisciplinary team and possess excellent written and verbal communication skills. What you need: Bachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW) Eligible for ordinary membership of AASW if overseas trained ( http://www.aasw.asn.au/ ) A current Victorian Driver’s Licence In Return: A supportive and dynamic team environment.
