About the role The Social Work team provides patients with additional support to improve psychological and social functioning and we ensure each patients social, emotional and cultural needs are met. We minimise the negative impacts of illness and hospitalisation on patients. We are a dynamic and innovative group who is always willing to share knowledge and peer support. There are regular department in-services, external professional development opportunities, you are able to access study leave for higher education. Key Responsibilities Ensure the smooth transition of clients through the health care system, liaising with relevant staff and community agencies to ensure continuity of care for individual patients / clients and their families Participate and fulfil requirements of allied health and discipline specific orientation, induction, mentorship, supervision and performance management programs Skills and experience Eligibility for practising membership of Australian Association Social Workers Membership (must have a bachelor or qualifying master’s degree) Demonstrated ability to monitor, evaluate and modify own performance through reflective practice Culture & Benefits At Grampians Health, we recognise our staff are our greatest asset.
