About the Role As a Health Care Worker with the Home Care Packages team, you will provide a high level of personal care and assistance to clients in their own homes within Port Fairy and surrounding district. You will provide assistance to clients as directed by their care plan, maintaining their independence and enhancing quality of life. About You To be successful in this role you must meet the following criteria: Certificate III in Individual Support (Ageing, Home & Community) OR Certificate IV in Aged Care Genuine desire to provide support and assistance to community members in their homes Knowledge of issues relating to frail/aged, people with disabilities, and their carers Knowledge of the wellness and re-ablement approach to aged care with regard to goal setting and care planning Understanding of Accreditation Standards, quality systems and continuous improvement models Demonstrated written and verbal communication skills Computer literacy About Us Moyne Health Services is a public, not for profit, integrated healthcare service that helps meet community healthcare needs in Port Fairy and surrounding districts in the Moyne Shire.
