With our focus on providing patient-centered care, our Social Workers play a crucial role in supporting patients in hospital settings. This casual role is perfect for someone looking to return to the workforce or seeking a flexible work arrangement. As a Social Worker in our hospital, you will be responsible for providing emotional support, counselling, practical assistance to patients and their families and ensure a smooth discharge. You will work closely with the healthcare team to ensure that patients receive the best possible care and support during their hospital stay. Key Responsibilities: Provide emotional support, counselling, and practical assistance to patients and their families. Work closely with healthcare professionals to identify patient needs and develop appropriate care plans. Conduct assessments and coordinate appropriate community resources for patients who require ongoing support. Develop and implement discharge plans for patients, ensuring a safe and smooth transition to home or a community setting. Provide education and support to patients and families regarding hospital policies, procedures, and available resources. Your Skills and Experience A bachelor's or master's degree in social work A minimum of 1 year of experience working as a Social Worker in an acute hospital setting Eligible to be an accredited member of AASW. Excellent interpersonal, communication, and problem-solving skills. Ability to work collaboratively with healthcare professionals and community agencies. Ability to work independently, manage time effectively, and prioritize competing demands. Ability to work a flexible schedule, including weekends and evenings as required.
