About the Role Clayton Social Work Department is looking for an experienced Grade 2 social worker to join the team working in the Palliative Care Unit at McCulloch House at Monash Medical Centre. This part-time (0.6 EFT or 22.8 hours per week), permanent role, is part of a flexible team who collaborate to provide a holistic approach to service delivery in a palliative care setting. This clinical role provides an opportunity to showcase your skills in working with people from a range of diverse backgrounds as well as completing complex psycho-social assessments and providing specialist bereavement, counselling and end-of-life care. This role professionally and operationally is managed by the Social Work Department and is located within the bed-based team with a focus and specialisation in palliative care and requires participation in the Emergency Department and on-call rosters. Located at Monash Medical Centre in Clayton, travel is also required across other Monash Health sites. Deliver and drive Monash Health strategic intent to be relentless in our pursuit of excellence across Social Work and Allied Health. Work closely with the Social Work Manager to provide streamlined services across all campuses and lead designated services/programs/portfolios. Travel is required across Monash Health sites. About You You will be a dynamic, innovative and flexible social worker with a good understanding of the treatment and psycho-social issues facing individuals and their families during end of life care. You will also have an understanding of the aged care sector, chronic health conditions and related services and referral pathways. You will be able to demonstrate skills in psycho-social assessments, problem solving, communication and hospital discharge planning. You ensure that high quality Social Work services that place the client at the centre of are provided. Education, quality improvement and evidence based practices will be your key drivers. Due to your proven clinical experience, you will bring knowledge of healthcare reforms and initiatives and challenges that public healthcare is facing now and in the future. What you need: You must be eligible for AASW membership including continuing professional education Proven experience in providing social work in acute and subacute settings. Knowledge and experience working closely with inter-professional teams. Current Driver’s Licence.
