This role works within a multidisciplinary team to provide Social Work assessment and intervention for clients in the community on the Transitional Aged Care Program and includes Case Management of clients as they progress through the 8-week program. This role is involved in quality improvement projects within the Transitional Aged Care Team, liaises with community services/groups and health professionals to assist clients to achieve their TACP goals, and contributes to the education of the Community Aged Care Team and other community teams as required. About you: We are seeking a skilled and energetic Social Worker who possess a passion for person centred care. Evidence of Tertiary Qualification in Social Work and eligible for membership of the Australian Association of Social Workers (AASW) Evidence of clinical experience in a hospital or health related field as a practising Social Worker. Current NSW drivers' license Class C or above. Demonstrated ability to work autonomously and as part of a multidisciplinary team. Demonstrated effective communication, consultation, and interpersonal skills. Demonstrated commitment to improving clinical practice and service delivery through leadership of quality improvement activities and ongoing professional development.
