See more at our website - www.atsichsbrisbane.org.au Job Description About the Role As a key member of the ATSICHS Brisbane team, the Social Health Care Coordinator is expected to personally contribute to the shaping and achievement of ATSICHS vision and goals within their own scope. The Social Health Care Coordinator will effectively assess, triage and refer clients who present, or are referred to Social Health. They will effectively manage and coordinate the service delivery model of case managed mental health wellbeing, alcohol and other drugs and social needs of clients. As well as contribute and participate in a diverse and multi-discipline team that promotes a flourishing workplace culture and upholds the ethos and values of ATSICHS Brisbane as a long-standing Aboriginal Community Controlled Organization. Duties Provide screening and assessments with client’s referred to Social Health in determining the support required. For clients identified for case management, deliver comprehensive assessments, determine a goal-oriented case management plan and deliver targeted supports to a wide range of clients and presentations, as per the case plan. Appropriately respond to critical incidents, providing wellbeing first aid to clients in distress. Provide high quality case management practices. Liaise with other community organisations and government agencies for the purposes of information gathering, referral, networking and promoting ATSICHS Brisbane Services. Benefits 12 Months Contractual Full Time position with annual salary of $70,537 to $77,234 (Depending on your Experience and Qualifications) 11% Super Not for Profit Salary Packaging (Increases your take home pay). Desired Skills and Experience Required Bachelor qualification in either Social Work/Human Services/Behavioural Science/Social Science/Psychology/Criminology/Allied Health/AOD/Occupational Therapy/Sociology/Public Health or relevant field. Demonstrated ability to apply a high level of practice judgement when working with clients in setting and achieving goals. Ability to work with a culturally and socially diverse group of clients, and to apply a culturally responsive framework to guide interventions, with the support of staff and management. Ability to work with diverse people and presentations in providing case managed social work driven supports. A willingness to travel across the ATSICHS Brisbane region of services and work across different sites. A willingness to provide social work supports and engagement to patients outside of a clinic environment. Working with Children’s Card (Blue Card) or be eligible to apply.
