We are seeking a qualified Social Worker to join our Allied Health team at Ramsay Health Plus - John Flynn Private Hospital on a casual basis. About the Role In this role you will provide high quality, direct social work services to both inpatients and outpatients of the hospital. You will respond to referrals for patients across the acute and physical rehabilitation inpatient setting. The social work department provides personal counselling, mediation and therapeutic interventions supporting patients and their families to adjust to diagnosis, trauma and emotional responses to illness and treatment. We provide assessment, support and advocacy for; end of life pathways, Palliative care, grief, loss and bereavement support, conflict resolution and advocacy support for patients. Knowledge and experience within NDIS, Guardianship applications and processes and community supports such at Transition Care program is an advantage. The Social Work team are involved in complex multidisciplinary discharge planning that includes collaboration and partnership with internal and external stakeholders to meet patient and family needs. We work collaboratively in a multi-disciplinary team to deliver quality patient outcomes. In this role you will report to the Allied Health Manager and work with a diverse team of allied health professionals. About You To be successful you will be a compassionate professional with excellent communication skills. You will demonstrate good professional reasoning and sound judgment in decision making considering implications and long term patient impact. Through your hard working and enthusiastic nature you have the ability to adapt to changing settings and shifts during the business week. You will strive to deliver quality patient care and exceed our customer expectations. You will also demonstrate; Degree in Social Work and membership with or eligibility for membership with the AASW. 1 year minimum experience in a similar role. Experience in undertaking Psychosocial assessments and risk assessments around elder abuse and exploitation Experience in working in a multidisciplinary team Experience in a Hospital or Aged Care setting is ideal, but not essential for experience candidates. Demonstrated ability to work independently and exhibit professional and ethical responsibility when working as a member of a diverse clinical team. Proven ability to work independently within a diverse clinical team. Demonstrated excellent organisational skills and effective communication skills.
