About the Role: An exciting opportunity is presented to work within the Northern Adelaide Local Health Network as a Social Worker within the Aged Care, Rehabilitation and Palliative Care Division. We are recruiting to multiple ongoing and temporary positions across the expanding Aged Care, Rehabilitation and Palliative Care (ACRPC) Division. Within Geriatric Services, there are opportunities across the Geriatric Evaluation and Management (GEM) Unit, Care Awaiting Placement (CAP) and the Transitional Care Program (TCP). Opportunities are also available within the Palliative Care Service. The Social Worker – ACRPC is accountable to the Adv/Nurse Unit Manager, Clinical/Senior Manager for the provision of social work assessment for the population of NALHN, which aims to optimise health outcomes. As a Social Worker you will apply clinical experience, increasing clinical knowledge and professional competence to plan, implement and evaluate comprehensive and integrated services to the needs of clients of NALHN. In this role you will work under limited direct supervision and may provide direction and support to less experience social workers, allied health assistants and students and may rotate through ACRPC clinical services. The Social Worker - ACRPC works within multidisciplinary teams in inpatient, community, and outpatient settings. The overarching role is to provide a comprehensive holistic service to patients / consumers and their families, with matters that may impact upon their social and emotional wellbeing during their involvement with ACRPC. Social work interventions are informed by thorough assessments and enhance a person’s potential to secure an appropriate care pathway and ensure patients / consumers are provided with access to appropriate, up-to-date resources and services to make informed decisions. Social Workers in ACRPC have diverse roles and support client cohorts in speciality areas, respect the diverse nature of the social work role and the specialty areas within ACRPC, including but not limited to: Palliative Care: Provision of social work support to Palliative Care patients, their families and carers across illness and bereavement phases. Geriatrics: Provision of social work support to older adults, families, and carers, going through life transitions, through an understanding of geriatric conditions and best practice guidelines around management, assessment, intervention, case management and referrals (e.g. ACAT, MAC, TCP and SACAT). Rehabilitation: Provision of social work support to adults participating in rehabilitation. This may involve assessment and intervention for adults going through complex life transitions, grief and loss and needing support for complex psychosocial situations. Specialised assessment based on diagnosis (eg stroke and amputee). Referrals to services including NDIS, SACAT, ACAT, MAC and TCP. To be eligible for this position, you must hold an appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
